With the forms below you will be able to catalog all anomalies and occurrences. These forms are in a PDF format and will require Adobe Acrobat to be viewed. They may appear distorted on some monitors at anything less than 150% enlargement but will print just fine.
These forms were created by the Paranormal Investigators Coalition (PIC) in order to promote a higher level of standards amongst ghost investigators. If you use them, please acknowledge where they were obtained from.

| The Investigator Cover Form This form is your cover sheet and will have your team member's names, base readings, number of visits to location, name and address of location, and a space for notes. |
Room Grid Form This form will allow you to document the location of hot spots and cold spots, artificial EMF locations, locations of furniture and obstructions and the room's reference point toward magnetic north. |
Area Grid Form This form is the same as the Room Grid Form except it is more detailed for outdoor investigations such as in cemeteries and battlefields. Mark the location of tombstones, monuments and trees. |
Photo Log Form This form will allow you to document where, when and why a photo was made. Who took the picture and if it was a new roll of film is also mentioned. We recommend using a 35mm camera. |
Video Log Form This form will help you with video surveillance. It will log up to 5 cameras (more if you use more than one form). It documents what type of camera, format of video, length of tape, location of camera, number of tapes used and reason for observation. |
Room Data and Information Form 1 This form will help you document room temperature and humidity, EMF levels, electrical field readings and RF levels. Also documents the location in each room the readings were taken and at what time. |
Room Data and Information Form 2 This form is much like the Room Data and Information Form 1 except it records cold spots, "strange feelings" (feeling of being watched, etc.) and "Other" readings with different types of devices. |
